Dear UMass Student-Athletes, Parents and Guardians,

I have enclosed information regarding the UMass Athletic Department Health Insurance Policies for the 2008-09 academic years and the drug-testing program for UMass intercollegiate athletics. All forms and a copy of the athlete's insurance card (front and back) must be submitted by August 1, 2008. If not submitted, the athlete will not be cleared for practices at the beginning of his/her season.

All student athletes are required to have primary insurance that covers athletic injuries to participate in Intercollegiate Athletics. If your son/daughters' insurance does not contain athletic injury coverage, medical care outside of your local area, or lapses for any reason, you will be responsible for paying all bills associated with that athletic injury. There is a secondary plan that can be purchased for a nominal fee that will cover all athletic related injuries at 100% coverage.

The Department of Athletics wishes to convey to the parents and guardians of incoming student-athletes a supplemental secondary insurance plan is in place and will become effective after an athlete's primary insurance company has processed and paid for all coverable services. In regards to primary health insurance, you must verify with the Sports Medicine staff that you fall under one of the following categories:

1. The student athlete has primary insurance coverage through their parents or guardian that covers athletic related injuries.

2. The student athlete has primary insurance coverage through their parents or guardian, but it does not cover athletic injuries or medical care outside of your local area. The student athlete must purchase an excess plan through K & K Insurance Services that will cover athletic related injuries. The premiums for this plan can be found on the Athletic Training Website on http://umassathletics.cstv.com. You can purchase this excess plan by contacting Kathy Boyd at kaboyd@admin.umass.edu or calling 413-545-4093 to help make arrangements for payment.

3. The student athlete does not have primary health insurance through their parents or guardian and must purchase primary health insurance. One option is purchasing the Student Health Insurance Plan (SHIP) through the University.

4. SHIP does not cover athletic related injuries and therefore if the student athlete purchases SHIP, they must also purchase an excess plan through K & K Insurance Services that will cover athletic related injuries. Please see #2 above for directions on premiums. Please view the Health Insurance Policy information on the http://umassathletics.cstv.com website. This will explain the different costs of insurance purchased through the University.

THE DEPARTMENT OF ATHLETICS WILL NOT PAY FOR MEDICAL COSTS FOR NON-INTERCOLLEGIATE ATHLETIC RELATED INJURIES OR ILLNESSES.

The Department of Athletics is NOT RESPONSIBLE for any non-sport related medical charges or a charge incurred for an athletic injury if the primary insurance is invalid or has lapsed. Please confirm with your insurance company that your son or daughter is covered as a college student, coverage is available for out of area care (if Mass not your local area), and your policy is up to date.

The University of Massachusetts, Amherst Athletic Department has instituted a drug-testing program for intercollegiate athletes. All student athletes must sign a consent form in order to participate in intercollegiate athletics.

This downloadable consent form is on-line at the UMass athletics web site. The drug testing consent form must be signed by the student athlete (or parent/guardian if athlete is not 18 years old) and turned into the Sports Medicine department before the student athlete is able to participate in varsity sport practice.

Please go to http://umassathletics.cstv.com and click on Athletic Training Services under the UMass Athletics section on the left side of the page. Under Student Athlete information you can view and print all of the instructions and forms that need to be submitted to the Sports Medicine Department. Please print and return completed Health Insurance Verification Form and Drug Testing Consent Form to the Boyden Athletic Training Room by August 1, 2008.

The student athlete must also submit a copy of both sides of their insurance card to the athletic training room before August 1, 2008.
